LPDB Complaint Form

If you would like to file a complaint against a public defender in Louisiana, LPDB has provided a CLIENT COMPLAINT FORM for your use.

The form must be printed, completed in writing, and mailed to the appropriate office. Unfortunately, we cannot accept complaints submitted electronically. We encourage you to read the LPDB CLIENT COMPLAINT POLICY before submitting your complaint.

If you wish to submit your complaints to LPDB, we will receive and redirect it appropriately. However, the fastest way to submit a complaint is directly to the appropriate District Defender or Program Director. You can view the contact information of all District Defenders by visiting our list of DISTRICT OFFICES or view the contact information of all Program Directors by visiting our list of CONTRACT PROGRAMS.
